# Hướng dẫn nộp tờ khai FATCA

Trong vòng 30 ngày kể từ khi mở tài khoản, Khách hàng vui lòng cung cấp biểu mẫu thuế của Hoa Kỳ theo hướng dẫn như sau:

**Bước 1:** Khách hàng thực hiện tải biểu mẫu tương ứng và thực hiện khai báo theo hướng dẫn tại website <https://www.irs.gov> (chi tiết tại <https://www.irs.gov/businesses/corporations/fatca-related-forms>).

*(\*) Khách hàng vui lòng cung cấp cho DNSE biểu mẫu W-9 nếu là cá nhân/tổ chức Hoa Kỳ, biểu mẫu W-8BEN nếu là cá nhân phải báo cáo khác và biểu mẫu W-8BEN-E nếu là tổ chức phải báo cáo khác.*

**Bước 2:**  Khách hàng thực hiện kiểm tra tính chính xác của thông tin khai báo, in, ký và gửi bản cứng về DNSE theo 01 trong 02 địa chỉ sau:

* **Tại Hà Nội**: Công ty CP Chứng khoán DNSE Tầng 6 – Tòa nhà Pax Sky, 63-65 Ngô Thì Nhậm, Hai Bà Trưng, Hà Nội ☎ 0247.1089.234
* **Tại TP. HCM**: Công ty CP Chứng khoán DNSE C-00.01, Tòa nhà Sarina, 62 Hoàng Thế Thiện, Phường An Khánh, TP Hồ Chí Minh ☎ 0287.105.9988
